Joseph Mengele (Gregory Peck) lebt ungestört in Paraguay im Exil. Er hat eine Gruppe ehemaliger Nazis um sich geschart und arbeitet an einem mysteriösen Projekt. Der junge Amerikaner Barry Kohler (Steve Guttenberg) kommt ihm, zusammen mit dem in Österreich lebenden Nazijäger Ezra Lieberman (Laurence Olivier), auf die Spur.
