Das Leben angehender Ärzte ist nicht leicht. Das stellen auch der im Mittelpunkt der Serie stehende John "JD" Dorian und seine Freunde Chris Turk und Elliot Reid Tag für Tag im Sacred Heart Hospital fest. Der Oberarzt Dr. Bob Kelso ist ein teuflischer Tyrann, der Hausmeister hat es vom ersten Tag an auf JD abgesehen und Dr. Perry Cox ist zwar JD's und Elliots Ansprechpartner, zeichnet sich aber eher durch unbarmherzigen Sarkasmus aus.
